Census: More Kansans lack health insurance | Wichita Eagle

Ask Susette Schwartz whether she?s surprised by a new report that shows thousands more Kansans going without health insurance, and she sighs.

?Well, no,? said Schwartz, CEO of Hunter Health Clinic in Wichita, a safety net clinic that serves uninsured and underinsured people as well as those with insurance.

?It?s just horrendous,? she said. ?Our number of patients has gone up, and keeps going up, and we just can?t keep doing this.?

The U.S. Census Bureau on Wednesday released estimates of health insurance coverage for every state and each of the nation?s nearly 3,200 counties.

The report showed that more than 380,000 Kansas residents under age 65 ? about 15.8?percent of that age group ? did not have health insurance in 2010, up from 14.6?percent the previous year.

For Wichita, the report was even more grim. More than 17?percent of Sedgwick County residents under 65 ? nearly 75,000 ? did not have health insurance in 2010, the latest year data is available. That?s 10,000 more uninsured people than the previous year and nearly 30,000 more than 2005, according to the report.

?I think we all know, with the economic downturn, that a lot of folks that did have employment are now unemployed or underemployed,? said Dave Sanford, CEO of GraceMed, another network of safety net clinics.

?We saw our demand increase several years ago, and it really hasn?t stopped,? he said. ?One of the challenges we have is continuing to keep up with the demand for care.?

Since 2005, the census report has broken down the statewide estimates by age groups, gender, race and Hispanic origin. For the first time this year, the report estimates health coverage at the county level for those in the 50 to 64 age group.

That inclusion ?allows tracking ? of this population, which is more likely to consume health care compared with younger age groups,? the report said.

Statewide in 2010, about 60,800 people ages 50 to 64 ? 11.4?percent ? did not have health insurance.

Local numbers mirrored the state: In Sedgwick County, about 12?percent of 50- to 64-year-olds did not have coverage in 2010; in Butler County, about 10?percent; and in Harvey County, about 11?percent.

The estimates are based on statistical models combining data from a variety of sources, including the American Community Survey, Census Bureau population estimates, federal tax returns, Medicaid participation records and 2000 Census statistics.

For Schwartz, the Hunter Health Clinic executive, all those numbers add up to more people at the clinic?s five sites in Wichita. Last year, Hunter Health served about 36,000 people with about 84,000 patient visits, she said. This year, the number of patients is more than 41,000.

About 70 percent of those patients are uninsured, Schwartz said.

?We?re still doing everything we?ve always done,? she said. ?We?re just running out of resources, that?s the bottom line.?

Should President Obama?s national health care law go into effect ? particularly a portion that would expand the Medicaid program for low-income Americans ? about 20,000 of Hunter Health Clinic?s patients would have insurance, Schwartz said.

More importantly, she said, those patients likely would get more regular and preventive care that could keep them out of more costly emergency rooms.

?When we have this many people uninsured, we?re not saving money, we?re spending it,? Schwartz said. ?If they?re not getting the prevention they need, they?re going to be in the hospitals, and that is so much more expensive.?

About a year ago, GraceMed acquired the Mother Mary Anne Clinic, an after-hours urgent care clinic for the uninsured and underinsured launched by Via Christi in 2008. The expansion allowed GraceMed to provide immediate care to more patients, as well as helping them deal with chronic conditions and receive follow-up care, Sanford said.

A project of the United Methodist Church, GraceMed also operates clinics at some Wichita public schools that provide low-cost medical and dental care on a sliding scale. Sanford said the longest wait times these days are for dental care.

?We secured one new dentist since last year, but whenever we have a new provider, word gets out and the demand gets that much more,? he said.

Schwartz, the Hunter Health executive, said Wednesday?s report from the Census Bureau illustrates a growing need for low- and no-cost health care, a demand she expects will keep going up along with the number of uninsured residents.

?We?ll just continue to do everything we can, but gosh, it?s hard.?

To watch Condoleezza Rice, the face of George W. Bush?s foreign policy, stand before a convention of cheering Republicans and condemn Barack Obama for diminishing America?s standing in the world?one can only gasp at the magnitude of chutzpah in one woman.

After describing a world of freedom fighters and tyrants, Bush?s former secretary of state and national security adviser lamented, ?Everyone asks, where does America stand?? Indeed that is the question of the hour,? she continued, for when ?friends or foes alike don?t know the answer to that question, unambiguously and clearly, the world is likely to be a more dangerous and chaotic place ? We cannot be reluctant to lead, and you cannot lead from behind.?

The solution, she concluded, is to elect Mitt Romney and Paul Ryan. ?They know what to do,? she said. ?They know that our friends and allies must again be able to trust us.?

First, it?s not at all clear that they have the slightest idea what to do or even?judging from Romney?s disastrously tin-eared trip to Europe this summer?how to think about what to do.

Second (this is the chutzpah part), Condi Rice?a top adviser in the most disastrous, reputation-crippling foreign-policy administration in decades?has no business lecturing anybody on this score.

Finally, Obama has done pretty well in foreign policy, and polls of actual foreigners?including allies?suggest that they think so too.

True, his foreign ratings?and America?s?have slipped a bit since the halcyon days when he first took office, but for the most part they?re still considerably higher than those of the Bush era.

According to a recent survey by the Poll Research Center, 53 percent of British citizens had a favorable view of the United States in 2008, the last year of Bush?s presidency. Today the figure is 60 percent. In France, the figure rose from 42 percent to 69 percent; the Czech Republic, from 45 to 54 percent; Germany, from 31 to 52 percent; Japan, from 50 to 72 percent; Mexico, from 47 to 56 percent. Only in the Arab countries (Lebanon, Egypt, Jordan) has the rating declined (and do the Republicans really care much about that?).

Another Pew Poll, released just this week, about global attitudes toward President Obama as a leader makes Rice?s concerns seem ridiculous. As summarized by CNN, 87 percent of the Germans, 86 percent of the French, 80 percent of the British, and 74 percent of the Japanese have confidence in Obama?in each case, more confidence than they have in their own leaders. More striking still, 92 percent of the French, 89 percent of the Germans, 73 percent of the British, and 66 percent of the Japanese want Obama re-elected.

What does Condi Rice know about Obama?s reputation in the world that the citizens of the world don?t?

Earlier in the evening, Sen. John McCain, Obama?s GOP opponent in the 2008 election, voiced the same concerns more grumpily. (Rice, it must be said, was eloquent, if a bit nervous at the start, and did not descend to the crowd?s rabidity; her most passionate plea, in fact, was for better schooling of K-12 youth.)

    "A GREAT Windows Utility"

    Pros

    I have been using Windows 7 manager for a long time, and prior to that, Windows Vista Manager. This utility has many, many custom Windows settings, tuning utilities and backup features. I backup my system registry daily with Windows 7 Manager and have used it to restore my registry many times. It works flawlessly.

    Cons

    None that I have found. It has NEVER caused me to lose data or cause any files to be deleted accidentally.

    Summary

    This is one great system utility. Well worth the price and I recommend it highly.

    Okay, everyone on here that seems to rate these things doesn't really know what they are talking about. I was directed here from a forum post while tweaking my registry manually.

    I did the dumb thing of downloading this and allowing it to do nothing but file extensions, not touching DLLs or system files. After install things on the computer were running horribly slow. Start up is crap, etc.

    Fact of the matter is DO NOT EVER DEFRAG or "OPTIMIZE" your registry with a program! There are EXTREMELY dangerous things to mess with. So, if you're not willing to get your hands dipped into the registry leave it alone.

    Any/Every automated registry cleaner IS BAD! May not be the first run or second, but trust me you are causing more damage to your operating system and things will get worse and worse over time.

    "makes a lot of things accessible in one place"

    Pros

    this app is really darn handy and time-saving: although it does many things you can already do in Windows 7 (if you're handy), this just makes it easier/quicker to find tweaks you need. I've used it for a year already.

    Cons

    But as with any or all of these types of utilities, a word of caution is necessary. Make sure you know what you're doing when you make any changes. You could end up with a messed up computer, like one guy in previous comments.

    How to Create an Injury-Free Workspace | Greatist

    Every morning I settle into my chair, sit up nice and straight, place my fingers piano-player-perfectly on the keyboard, and start working. By about 3:00 pm my graceful posture has deflated into a slight slouch; by 4:30 pm I?m lucky if my rear end hasn?t fallen off the seat cushion.

    Mid-afternoon contortions are pretty common among today?s employees. In 2010, musculoskeletal disorders made up 29 percent of serious workplace injuries in the USA, and there?s evidence that the number of employees twisted like pretzels is increasing.

    It?s time for an intervention, and for good reason: Wacky positioning in a poorly organized workspace can contribute to injuries such as carpal tunnel syndrome, muscle strains, and vision problems[]. But slouch and strain no more. We?ve got all the tips for designing an ergonomic workspace ? the fancy term for a work area that?s safe, comfortable, and good for our health. Read on for a how-to lesson in modern workplace safety.

    Office Space ? The Need-to-Know

    Rock climbing, windsurfing, and tackle football ? now those sound dangerous. But sitting at a desk for even part of the day can be just as risky. Common office injuries include carpal tunnel syndrome (nerve damage in the wrist) and myofascial pain syndrome (chronic pain disorder) of the shoulder, often caused by incorrect typing and mousing techniques[]. Muscle strains in the neck, back, and shoulders can happen when we?re hunched over a keyboard, say, when inputting numbers into an Excel sheet.

    It?s not just muscles, either. Staring at a computer screen for too long can cause headaches and swollen, irritated eyes. If any of these symptoms sound familiar, there are thankfully some easy ways to avoid workplace health issues.

    Ergonomicize ? Your Action Plan

    Some research suggests an ergonomic setup can alleviate symptoms of musculoskeletal disorders[]. But before calling in Extreme Makeover: Office Edition, know that it?s still unclear whether ergonomic interventions can help carpal tunnel syndrome and other similar issues[][]. In some cases, an ergonomic redesign may not be enough, and medical attention might also be necessary[].

    If muscle pain or vision problems are causing serious issues, it might be time to see a doctor. The steps below won?t reverse damage but can make for a spiffy, safer, Greatist-approved workspace.

    • Chair: There?s no need for a bejeweled royal throne, but the seat cushion should be comfortable and the chair should have armrests that allow elbows to rest at a 90-degree angle while typing. And one size does not fit all butts, so make sure it?s possible to adjust the height of the seat so arms are at the height of the desk, thighs are parallel to the floor, and feet rest flat on the floor. The backrest should be at a 90-degree angle to the desk (or farther forward depending on personal preference). Swiveling capabilities are also a plus so we don?t need to stretch and strain to reach the stapler in the corner of the desk. Or mix things up and try a sit-stand workstation, which some employees say reduces back and neck pain and helps keep them focused during the workday.
    • Desk: The desk space should be large enough to accommodate all the essentials ? monitor, keyboard, mouse, notebook, water bottle, etc. For those who use a mouse instead of a track-pad, position the mouse and keyboard close together. When you look down, the ?B? key should be right under your nose. And no fancy dance moves at the desk, either: Wrists should be in a neutral position, not extended up or down. It can also help to use a wrist rest, or a simple cushion that can help prevent contact stress from pressing wrists into the table edge.
    • Monitor: The computer screen should sit about an arm?s length away from you, and should face directly toward you (not angled to the right or left). To minimize glare, don?t wear shades, but align eyes with the top of the screen?s viewing area. Follow the 20/20/20 rule: For every 20 minutes spent staring at Pinterest (er, a Word Doc), take 20 seconds to look away from the computer at something at least 20 feet in the distance. Can?t spare 20 seconds? Try taking a gym break or lunch hour away from the computer to give eyes (and mind) some reprieve.
    • Mouse: Consider using a trackball or a touchpad to minimize the repetitive motions of moving a mouse. For standard mouse fans, choose a mouse that fits the shape of your hand but is still as flat as possible. Some people prefer to forgo a mouse pad, which can place the mouse at a higher angle and cause wrist strain. And this isn?t a Frisbee game: Move the mouse from the elbow and not the wrist.

    Senior Care Danvers MA: Elder Care Legal and Financial Checklist

    Preparing your legal and financial situation properly is not always an easy or pleasant task but it is an important one for seniors and Senior Care Danvers MAtheir family to complete. Working through a comprehensive list of considerations can help make the task easier and will identify resources and decisions about senior care and your property.

    Durable Power of Attorney

    Durable power of attorney is a legal document naming someone to act on your behalf if you are unable to make decisions about your own care, either mentally or physically. This person has legal permission to pay your bills, direct your medical care and make other financial decisions for you.

    There are two types of durable power of attorney:

    • ?Medical-Durable power of attorney for health care allows a selected person to make medical choices on your behalf if you are not able to do so. This person will be the point of contact for your doctors and other medical personnel.
    • ?Financial-Durable power of attorney for finances is the person who has authority to make financial choices and transactions on your behalf. This may include power over your real estate, social security, investments, tax returns and paying your bills.

    Note that you do not have to select the same person as your durable attorney for medical care and finances but you certainly may. However, it is a good idea to create separate documents naming the durable power of attorney for health care and one for the durable power of attorney for finances. This helps keeps the issues separate and provides a clear directive for your family and others.

    Living Will

    Also known as an advanced care directive, a living will provides instructions for your care if you are unable to make decisions on your own. Establishing a durable power of attorney for medical decisions is often part of creating an advanced care directive.

    In your advanced care directive, you can provide instructions regarding organ donation and your wishes regarding the use of CPR, prolonged use of a respirator, blood cultures and other invasive diagnostic tests, artificial nutrition through a tube or IV and/or blood transfusions. In addition, an advanced care directive may include a Do Not Resuscitate Order (DNR) if you stop breathing or your heart stops beating. A DNR states that you do not wish to have measures taken to restore your heartbeat or breathing if either or both stop.

    Spend the time thinking about how you feel regarding these issues and speak with your family about your wishes and theirs before completing the advanced care directive.

    Long Term Care Insurance

    Another important factor in elder care is long term care insurance (LTCI), which helps cover the cost of your medical and living expenses not covered by traditional health insurance, Medicaid or Medicare.

    Long term care insurance helps cover the cost of home senior care, adult daycare, nursing home, assisted living and other out-of-pocket costs associated with elderly care. Note that some insurance companies require a waiting period before the LTCI policy goes into effect. Check with your insurance company for more information.

    Income and Expenses

    Another important element in preparing for the costs of care and transitioning of financial control to family members includes writing down important financial information for your family to use as needed.

    This information should include the following:

    • ?Monthly income sources-Write down your regular sources of income, such as social security or pensions. Include your account information and policy numbers.
    • ?Monthly expenses-Write down your monthly expenses, such as your mortgage or rent, utility bills, cable, television, phone and any other bills that you are responsible for on a monthly basis. Include your account number and any passwords for online access.

    Will

    A will identifies the division of your property and assets after your death. Here are some of the basics to consider:

    • Property-Clearly identify who is to receive ownership of your property, whether it is a person or an organization. Be aware that property and estate taxes can be complicated, so contact a local lawyer to help you figure out how to pay the state and federal taxes and how to keep your property out of probate court.
    • Executor-The executor makes certain that the terms of the will occur according to your wishes.

    Trust

    A trust is an estate plan for minor children to whom you wish to leave money but want to manage its execution. Several elements will help avoid problems for those working to establish the trust.

    • ?Life insurance trust-An irrevocable life insurance trust will put the proceeds from your life insurance policy back into your estate while protecting the proceeds from federal estate taxes. Speak with your life insurance company about setting up an irrevocable life insurance trust associated with your life insurance policy.

    Protection

    Ensure that your money goes to minors as you wish by determining the amount of money that you wish for each minor child to receive each year. You may also determine an age at which the final lump sum will pay out to the child, such as after college graduation or age 25.

    In addition, you can ensure that the money goes to funding the education of minor children by specifying the use of the money, such as tuition or school fees.

    Charity

    A charitable trust can specify that some or all of your money is to go towards a charity or charities of your choice.

    While getting your financial situation and wishes in order can be time consuming, doing so will make life easier for you and your family as your health progresses or in case you are no longer able to make decisions on your own.

    Researchers are seeking overweight men for the study.

    Victoria University researchers are conducting health and fitness checks for overweight men as part of a study into blood sugar control.

    Lead researcher Dr Itamar Levinger said participants would receive free fitness assessments, bone density scans and checks on cholesterol, Vitamin D and blood glucose levels as part of the study. They will also have muscle biopsies ? involving small muscle samples from the thigh ? and insulin and glucose infusion to examine the muscle?s capacity to uptake sugar at rest and after exercise.

    ?These assessments are essential for understanding the connection between bone hormones and muscle function in people with elevated sugar level and people with type 2 diabetes,? Dr Levinger said.

    Dr Levinger explained that far from simply supporting our bodies, bones were integral in regulating our bodily functions through the hormones they produced.

    ?Preliminary data shows exercise increases the hormones produced by bone related to a reduction in blood sugar levels and muscle function,? he said. ?If we can understand this link better it will open new avenues to more effective treatments of diabetes and cardiovascular conditions that target the precise function without the adverse affects of some current treatments.?

    Participants in the study should be men aged between 40 and 70 years old, overweight (with a BMI over 25), have elevated blood sugar levels and not be on insulin or other medication that affects sugar levels. They will be required for four visits to Victoria University?s Footscray Park campus and one to The Austin hospital in Heidelberg.

    The study has been approved by Victoria University?s Human Research Ethics Committee.

    Participants will be reimbursed $300 on completion of the study for their time and travel costs.

    Taylor Swift's "Never Ever" breaks music chart record

    LOS ANGELES (Reuters) - Country star Taylor Swift's latest single made a record-breaking entry in Wednesday's Billboard Digital Songs chart, scoring the biggest digital sales week ever for a song by a female artist.

    Swift's song "We Are Never Ever Getting Back Together," an infectious, punky country-pop anthem, quickly rose through the charts and gained strong radio play after its release last week, selling 623,000 downloads, according to Nielsen SoundScan.

    The song also hit the Billboard Country singles chart, as Swift's music has attracted fans of both country and pop.

    "Taylor appeals to a younger female demographic that tends to buy a lot of music and after the years, she's become a staple of two pretty popular formats, country and pop, and she's able to manipulate both formats and be played across both," Silvio Pietroluongo, director of charts at Billboard, told Reuters.

    Pop singer Ke$ha's "TiK ToK" previously held the title for the biggest first-week digital sales by a female artist, with 610,000 downloads.

    Only male rapper Flo Rida's "Right Round," featuring Ke$ha, sits above Swift as the single with the largest first-week download sales, clocking in at 636,000 copies.

    The success of Swift's latest single - the first to be released from her upcoming, fourth studio album "Red" - sold almost double the number of her previous best-selling song, "Love Story," which debuted with 360,000 copies in the first week of its release in December 2008.

    Swift, 22, has won fans with her raw lyrics, and the singer has often said her songs are about real life experiences. She addressed her 2010 song "Dear John" to singer John Mayer after their relationship ended.

    Swift has also dated celebrities Joe Jonas and Jake Gyllenhaal and is currently dating a member of the Kennedy family.

    "We Are Never Ever Getting Back Together" has piqued the curiosity of Swift's fans over the past week as they try to uncover which one of her famous ex-boyfriends was the subject of the song.

    Cartoon stickers may sway kids' food choices: study

    (Reuters) - Can Elmo make children like apples?

    For children who turn up their noses at fruits and vegetables, slapping a cartoon face on a healthy snack may make those choices more appealing, according to a U.S. study.

    Researchers, whose findings appeared in the Archives of Pediatric and Adolescent Medicine, discovered that when elementary school students were offered apples and cookies with lunch, children were more likely to opt for an apple when it was branded with a cartoon sticker - such as one of the "Sesame Street" character Elmo.

    "If we're trying to promote healthier foods, we need to be as smart as the companies that are selling the less-healthy foods," said David Just, co-director of the Cornell Center for Behavioral Economics in Child Nutrition Program, who worked on the study.

    Noting that cartoon characters and flashy advertising often don cookie and candy packaging, he added, "The message should be: fight fire with fire."

    Just and his colleagues offered cookies and apples to 208 eight- to 11-year-olds at suburban and rural schools every day at lunch for a week. Children were allowed to choose an apple, a cookie or both, along with their normal meal.

    Some days, the snacks were offered without cartoon stickers or other branding. On other days, either the cookie or the apple was branded with a familiar cartoon character.

    When the snacks weren't specially marked, 91 percent of children took a cookie and just under one-quarter took an apple.

    But when an Elmo sticker was slapped on the apples, 37 percent of children took fruit, the researchers reported.

    Stickers on cookies didn't affect children's choice of the sweet snack.

    "There are so many foods that are of poor nutritional quality and they are being marketed to children," said Christina Roberto, who studies food choices at the Harvard School of Public Health in Boston and did not take part in the study.

    Kid-friendly characters used for this marketing "aren't popping up on the carrots and apples as much as they are on a wide range of foods that aren't good for kids," she added.

    Using stickers on fruits and vegetables could be one cheap option to help improve students' diets, she said, as well as something parents can try at home.

    USS Porter collides with oil tanker in Persian Gulf

    The U.S. Navy destroyer USS Porter suffered significant damage after colliding with an oil tanker in the Strait of Hormuz on Sunday.

    The USS Porter was passing through the Strait of Hormuz into the Persian Gulf.

    The Japanese-owned, Panamanian flagged tanker M/V Otowasan was coming the other way, just entering the Strait of Hormuz, and the two crashed into one another.

    The shipping lanes are very narrow, about two miles wide in each lane. The crash left a hole in the side of the destroyer but no injuries were reported.

    The damage is all above the water-line, so there is no danger that the ship will sink, but likely sustained water damage because the fire control system would have activated.

    The USS Porter is headed to a port a called Jebel Ali in the UAE.

    Familiar foe: In Ryan, Obama sees twofold target

    (AP) ? In his run for a second term, President Barack Obama had an opponent before he had an opponent: House Republicans. They shellacked him in the midterm elections, blocked much of his legislative agenda and pushed economic views that are wildly different from his.

    Mitt Romney put a campaign face on all that for Obama: Paul Ryan.

    Now Obama is attacking both the "do-nothing Congress" and Romney at once, two forces united as a target on the Republican presidential ticket. Going after votes in Iowa on Monday, Obama called Ryan the "ideological leader" of House Republicans and singled him out as "one of the leaders of Congress standing in the way" of a bill to help farmers in a time of disastrous drought.

    "I've gotten to know Congressman Ryan. He's a good man. He's a family man. He's a very articulate spokesperson for Governor Romney's vision," Obama said in Boone on the first of a three-day bus tour through Iowa. "The problem is it's the wrong vision for America. It's a vision that I fundamentally disagree with."

    Obama also has something with Ryan that he does not with the presumptive Republican nominee ? a relationship of sorts. Obama has laughed with Ryan, sparred with him and attacked his ideas right in front of him. Even their favorite football teams, Obama's Chicago Bears and Ryan's Green Bay Packers, are rivals.

    "I know him," Obama told supporters over the weekend. "I welcome him to the race."

    He meant that politically, not just politely, as both sides adjust to what Ryan means at Romney's side.

    Romney is benefiting from the energy, campaign buzz and ideas that come with Ryan, the 42-year-old rising political star from Wisconsin. Yet Romney also aligned himself with Congress, whose public approval of its performance was a lowly 22 percent in an AP-GfK poll earlier this summer, compared with nearly 50 percent for Obama.

    Ryan is chairman of the House Budget Committee. His name is ideologically synonymous in Washington with controversial plans for cutting spending.

    Long before Romney announced Ryan as his running mate Saturday, Obama himself sought to link the two men. The president sees Ryan as the budgetary voice of a "prescription for decline" in America in which government policies help the rich at the expense of everyone else.

    When Obama started going after Romney more directly in the spring, he specifically cited Romney's support for Ryan's budget-slashing blueprint, one that would overhaul Medicare and cut taxes and the deficit. Obama called it "thinly veiled social Darwinism" that would gut opportunity and upward mobility.

    At least Ryan was not in the audience that time.

    In a blistering speech in April 2011, Obama unveiled a plan for cutting the nation's debt in the long term, seen as a counterpunch to a plan from Ryan. Referring to the Ryan plan, Obama said it would "end Medicare as we know it," a message his campaign will now blast every day.

    Ryan said at the time he was excited to get invited to the speech at George Washington University. That changed when he heard Obama speak. "What we got was a speech that was excessively partisan, dramatically inaccurate and hopelessly inadequate to addressing our country's pressing fiscal challenges," he said.

    To audiences, Obama and Ryan can both operate in a big-message world, but they are both comfortable in the weeds of policies. Even with each other.

    When Obama spoke to House Republicans at their own conference in January 2010, he commended Ryan for having put forward "a serious proposal." Obama made clear it had ideas he agreed with and plenty of others "we should have a healthy debate about because I don't agree with them."

    The next month, at a health care summit, Obama and Ryan got into a wonky exchange about how to shrink costs to the taxpayer. When Ryan called Obama's vision a "government takeover of health care," Obama responded at length, and the two sounded like they could have debated the points all day.

    "There are some strong disagreements on the numbers here, Paul," Obama said, "but I don't want to get too bogged down."

    Obama has found himself at constant odds with Ryan and other House Republicans, and tried to make it a campaign approach. "I would love nothing more than to see Congress act so aggressively that I can't campaign against them as a do-nothing Congress," he said in October while pitching his jobs plan.

    That sounded a lot like his comments in Iowa on Monday, when he went after Ryan by name, which was his way of going after Romney.

    And, as Obama said, he knows Ryan.

    During that meeting of House Republicans in 2010, Ryan graciously thanked Obama for showing up to what was clearly a tough audience.

    "Good to see you," Obama told Ryan. "Is this your crew right here, by the way?"

    "It is," Ryan said. "This is my daughter, Liza, my sons, Charlie and Sam, and this is my wife, Janna."

    When the president then said a personal hello to the whole Ryan family, Ryan prodded them: "Say hi, everybody."

    And everyone laughed.

    Panetta: Pakistan military plans to open new front

    WASHINGTON (AP) ? Pakistan has told U.S. military officials that it plans to launch combat operations against Taliban militants soon in a tribal area near the Afghan border that also serves as a haven for leaders of the al-Qaida-affiliated Haqqani network, Defense Secretary Leon Panetta said Monday.

    Speaking to The Associated Press in his Pentagon office, Panetta said Pakistan's military chief, Gen. Ashfaq Parvez Kayani, discussed the planned operation in recent conversations with the top American commander in Afghanistan, Gen. John Allen.

    Panetta said he did not know when the Pakistani operation would start, but he said he understands it will be in the "near future," and that the main target will be the Pakistani Taliban, rather than the Haqqani network.

    Panetta welcomed Kayani's initiative, even though the main target may not be the Haqqani leadership.

    "They've talked about it for a long time. Frankly, I'd lost hope that they were going do anything about it. But it does appear that they in fact are going to take that step."

    Panetta also revealed that the U.S. is providing additional military assistance to peacekeeping forces in Egypt's Sinai Peninsula in order to strengthen security in the region. But he said that so far the Pentagon has not moved to send additional U.S. troops to the Sinai. A truck-mounted tracking system sent to the Sinai will allow troops to follow friendly forces.

    "We just want to make sure that we know how those forces are deployed in order to insure that we can more effectively go after those terrorists that would try to create an incident or terrorist act," Panetta said.

    The U.S. long has been frustrated by Islamabad's refusal to target Afghan Taliban militants and their allies using Pakistani territory to stage attacks against U.S. and coalition troops in Afghanistan. Many analysts believe Pakistan is reluctant to target groups with which it has strong historical ties and could be useful allies in Afghanistan after foreign forces withdraw.

    Touching on another major U.S. frustration in Afghanistan, Panetta said he saw the accelerating pattern of attacks on American and other coalition troops by members of the Afghan army and police as a sign that the Taliban is grasping for success. But he also said he has been assured by U.S. military commanders that "this still remains sporadic" and not a long-term trend.

    "But, having said that, we are keeping a very close watch on the situation," Panetta added.

    He argued that the Afghan insider attacks, in which numerous Afghan troops have turned their guns on coalition forces, are in some cases a reflection of efforts by the Taliban to use unconventional tactics against a coalition force that it cannot defeat on the battlefield.

    "The Taliban has not been successful at regaining any of the areas that (it) lost" over the past two years, he said. "And so I think part of what you're seeing is that they are going to resort to" a strategy of using unconventional attacks, including improvised explosive devices and the encouragement of insider attacks.

    "I think this is an indication that they are failing," he added.

    Panetta said he was comforted by President Hamid Karzai's decision to apologize for the most recent spate of insider attacks. Karzai condemned the killings, ordered investigations into the incidents and directed relevant Afghan authorities to work to ensure safety within training and security institutions.

    "The enemy who does not want to see Afghanistan have a strong security force, targets military trainers," Karzai said in a statement Saturday.

    Panetta, 74, spoke at length on a wide range of topics during an AP interview, with his golden retriever, Bravo, lying at his side playing with a red stuffed lobster toy.

    The Pentagon chief said relations with the Pakistani military have improved "a great deal" lately, after a falling out over American airstrikes that killed 24 Pakistani soldiers in November and Pakistan's subsequent closing of border crossings that facilitated the flow of military supplies to U.S. and coalition troops in Afghanistan.

    He said Allen and Kayani have "discussed concerns" about the Haqqanis, whose fighters have moved back and forth across the border to launch attacks in Afghanistan.

    "Gen. Kayani did indicate that they had developed plans to go into Waziristan," Panetta said. "Our understanding is that hopefully they're going to take that step in the near future. I can't tell you when. But the indication that we have is that they are prepared to conduct that operation soon."

    Pakistan has denied this, saying its forces are stretched too thin fighting Pakistani Taliban militants at war with the state.     Adrian Peterson has taken a giant leap toward returning from a torn ACL.

    The Vikings on Sunday removed Peterson from the Physically Unable to Perform list, which means that (barring a season-ending injury) he?ll be on the Week One roster.? But it doesn?t mean he?ll be playing when the Vikings host the Jaguars four weeks from today.

    ?I want to caution you, I know there are a lot of fans that are very optimistic and excited about seeing him back but for us it?s just a part of the process,? coach Leslie Frazier told the media, via comments distributed by the team.? ?It doesn?t mean a whole lot other than he?s done a great job in his rehab up to this point.? Off to the side with our strength coaches, with [trainer] Eric Sugarman and his staff and our medical staff explained to myself and [G.M.] Rick Spielman that the next step for him in the process would be to get integrated with his teammates and that?s just what it means.? He?s done everything he can do on the side, now it?s the next step in the process.? It doesn?t mean in the future he?s going to be lining up with our team in the opening game, we don?t know that.? We have a long time to determine that, this is just another step in the process.?

    Under the terms of the new CBA (which is being honored in most cities not located in Washington state), Peterson must complete a two-day acclimation period without pads.

    ?[T]oday along with Monday he can do some individual drills, he can do our walk through, but he can?t do any of the padded stuff that we?re doing and we?re in pads right now, so it won?t be until Tuesday when we can put him in pads and let him go through some of the padded work that we?ll do,? Frazier said.? ?We?ll just keep going through the process.?

    Peterson, who won?t be exposed to fully live contact during practice (Frazier pointed out that Peterson never is), embraced the opportunity to get closer to playing.

    ?I asked him, because we?ve been talking about this for a while,? Frazier explained,? ?I said, ?Now is your chance to say Coach, I was wrong, I don?t think I?m ready.?? He said, ?I?m not saying that, I?m ready I can?t wait to get out there.?? He?s pretty excited.?

    Frazier, who said Peterson did a good job of lobbying for the ability to return to action, wouldn?t say whether Peterson will play in the preseason.? Even if Peterson is able to suit up and go as of Week One, it doesn?t mean he?ll be the workhorse.? The Vikings could choose to go with Peterson and Toby Gerhart for most if not all of the year, given that it could take Peterson a full season to fully recover from his December 24 injury.
